Overall Comment
(Revised 05.06.2026 Inspection Report)
A re-inspection was conducted with Brookdale’s Maintenance Director,
James Thompson, for the items previously mentioned on the prior
inspection report. The following items appeared to have been corrected
at this time based on a visual re-inspection.
- The entrance doors for unit C1, B4 now have a fire rated certification
label.

- The entrance door to the kitchen has been replaced.
- The entrance door to enter the D wing now has a fire rated certification
label.
- The exit door by unit C4 & C5 now had the self-closing component
working appropriately as required.
Only remaining item is the following.
- The exit door by unit D5 & D4, was still missing the fire
rated certification label.
James was going to follow up on this item and reach back out to our
office once it has been properly corrected.

Overall Comment

this record. All previously noted items, with the exception of the exit door by
units D4 & D5, have been corrected. The Maintenance Director, James
Thompson, will notify our office when this item has been corrected.

@Christopher Torres What is the timeline associated with the violations,
which where identified on May 6, 2026 report? I would like to issue the
letter, but I am attempting to understand their timeline for the corrections.
