GENERAL INSTALLATION DISCLOSURE We make every effort to respect your landscaping and work with utmost care around it. Because of our extensive hose lines, please notify us of any especially valuable plants or items that you want us to pay close attention to. HALCO is not responsible for damage to any objects in the walls that are not pointed out by the homeowner. Examples are hidden circuit boards, uncovered electrical boxes, hidden wall cavities, hidden pocket doors, safes, unusual plumbing, recessed cabinetry, etc. Specify any unusual or hidden objects (write NONE if not applicable). ______________________________________________________________________________ If the installation of insulation results in any damage to your home due to the negligence of the installer, the installer's liability shall be limited to the amount paid by the homeowner to the installer. HALCO is not responsible for moving personal belongings. If HALCO needs to move any personal belongings there will be a charge of $50 per man per hour with a $100 minimum. We are not responsible for any items we must move. Homeowners are responsible for removing valuable items away from and off of walls and ceiling prior to the start of the project. Vibrations from work could cause these items to shift or fall. HALCO is not responsible for such damage. SOME IMPORTANT INSTALLATION FACTS CONCERNING RETROFIT FOAM INSULATIO N While drilling through the exterior walls and while installing the retrofit foam insulation the electricity to the entire house must be shut off until installation is complete. After installation, we must turn on electricity one circuit at a time. If any circuits do not function properly then they will need to remain off until the foam has cured. The homeowner understands to not tum on electricity to work areas until the job has been completed. Retrofit foam insulation is installed using air pressure, so any electrical wires that are loose could pull away from fixtures during installation. HALCO is not responsible for these items. Once installation is complete the electrical boxes will be checked before electricity is turned back on. If an electrical circuit does not function properly after installation we must wait until the insulation cures to determine the cause of the short. HALCO has disclosed all the above electrical issues and homeowner understands that an inconvenience may occur until the insulation has cured. HALCO is exonerated from liability of any pre-existing electrical conditions. Typical blind cavity fill is 98%. R-value of our retrofit foam insulation is 3.9/inch. Retrofit foam, in its primary stage, is a semi-liquid substance and it is highly likely that wood wall coverings will show signs of expansion and contraction during the curing phase. HALCO does not recommend installing retrofit foam insulation in direct contact with wood wall and ceiling coverings, i.e, paneling, wainscoating. etc., and is exonerated from liability regarding such applications. HALCO takes the utmost care handling and reinstalling your siding material. Aluminum siding is often difficult to remove and reinstall. It's likely that if your siding is aluminum, it will not look the same after reinstallation. If your aluminum siding has not been painted , it's also normal for the paint to come off from the pieces being worked on and may require repainting or touch-up, this is the homeowner's responsibility. If the siding has aged, our washing process may remove some paint causing discolo ration. HALCO is not responsible for flaking paint. For brick homes. 3/4 inch access and pressure relief holes are drilled through mortar joints . The bricks may be chipped during the installation process if you have thin mortar joints or weakened bricks. Utmost care will be taken to minimize this, however HALCO will not be held liable for cracked or chipped brick. Tinted mortar will be used to point access holes to closely match existing mortar. Because the integrity and structure of the walls being filled with retrofit foam is a factor not under our control , we cannot be responsible for interior or exterior sheeting, drywall , plaster or other wall coverings that may be in a weakened cond ition and not able to handle the pressure Please direct all inqueries regarding this project to the Home Performance Department at Halco by calling 800-533-3367 or email homeperformance@halcoenergy.com. ON-GOING CONTACT INFORMATION I have read, understand and accept the terms of this section . INITIAL (1) _________INITIAL (2) _________ *** If this project is to be fully or partially paid for by energy incentive subsidies, grants and/or financing, then the complete details of this project must be submitted to the sponsoring utility or company to determine compliance and conformance with all rules and applicable technical standards. If the sponsoring utility or company requires any modification or adjustments to this defined scope of work and/or cost participation, or if the sponsoring utility or company declines approval of this project, you will be notified of any such revisions or decisions and you will then have three business days to determine your commitiment to move forward with all or part of this project under revised and mutually agreeable terms of payment. SUBSIDY PROGRAM ACCEPTANCE AND APPROVAL DocuSign Envelope ID: 5011CD3C-7389-4E2E-A258-A036E05EBF8E 5/20/2022 cannot be responsible for interior or exterior sheeting, drywall , plaster or other wall coverings that may be in a weakened cond ition and not able to handle the pressureexerted by our insulation applicat ion. HALCO is exonerated from liability. HALCO is not responsible for any cracked or damaged plaster or drywall. Since we do not know the application method of the sheets, we can not be responsible for their structural integrity. Homeowner takes 100% responsibility for the plaster and drywall before and after installation. HALCO is not responsible for any damage caused by vapor flow transmission issues after retrofit foam has been installed. Vapor flow transmission is the movement of water vapor in the air through a material. This water vapor comes from the inhabitants activities, i.e. boiling water, bathing, drying clothes, etc. Vapor flows from hot to cold and condenses when the dew point is met. This vapor may cause damage to painted surfaces on the exterior of the home when the paint acts as a vapor barrier, not aHowing the vapor to escape. The vapor may then cause premature peeling of the paint. Thimble vents may be installed to help alleviate this problem . HALCO is not responsible for any damage that may be caused to the paint such as cracking, chipping, bubbling, flaking or peeling. Some exterior paints act as vapor barriers and some do not. Since HALCO did not paint the home we do not know if the exterior paint will act as a vapor barrier. Helpful Tips: We need a source of potable water to mix the product on site. Please notify your representative which spigot the installation crew should use. SOME ADDITIONAL IMPORTANT INSTALLATION FACTS CONCERNING RETROFIT FOAM INSULATION HALCO is not responsible for any personal property/belongings left in the areas where foam is being sprayed. The customer/contractor is responsible for removing any objects that they do not want foamed. If belongings can not be removed, the homeowner/contractor is responsible for covering all belongings so there is no possible way the foam (overspray) could land on objects . HALCO is not responsible for any damages to personal belongings from overspray because they were not removed or covered. HALCO recommends that all expensive belongings ( i.e. vehicles) be moved away from the area that is being sprayed. When spraying to exterior sheeting or soffitt areas, there is a potential for overspray to travel on air currents and be deposited on your property. HALCO will not be responsible for this damage. HALCO recommends that the homeowner/contractor cover exterior sheeting with a weatherproof product (i.e. Tyvek or siding). HALCD is not responsible for any overspray on areas that are not covered. HALCO is also not responsible for any foam insulation that is exposed to moisture. All projects are sold and installed to a specific depth average. All thickness are averages and may vary up to 1/2" . Customer/contractor is responsible to make sure all electrical wires, phone wires, cable wires, plumbing pipes, and heat runs are secure in the wall cavities. Objects may move while foam is expanding. HALCO is not responsible for keeping the items in the cavities . If they move outside the wall cavities, they may be damaged during the trimming process. HALCO will not be responsib le for this damage as items should have been secured into wall cavities before installation of insulation. As like any new construc tion product. the foam does have a mild odor. The odor will diminish over time. HALCO has no method to change the odor or make it dissipate any quicker. In order to eliminate thermal bridg ing, which is the conduction of heat from inside of the home to the roof deck through the rafters, we recommend a thermal break. The thermal break could be furring strips running horizontally, foam board, or any other product that will not allow conduction from the ceiling to the roof deck.
